What Is Trauma-Informed Care?
Trauma-informed care is an approach that recognises the widespread prevalence of trauma, integrates knowledge about trauma into all policies, procedures, and practices, and seeks to avoid re-traumatisation. Trauma-informed care is not a specific treatment — it is a framework that informs how any service, organisation, or individual provides support. Trauma-informed care shifts the question from "what is wrong with this person?" to "what happened to this person?" — a fundamental reframing that changes how distress and difficult behaviour are understood and responded to.
The prevalence of trauma makes trauma-informed care relevant across all health, social, and educational settings. Adverse childhood experiences (ACEs) research shows that over 60% of adults have experienced at least one ACE — abuse, neglect, household dysfunction — and ACE exposure is dose-dependently associated with mental health, physical health, and social outcomes across the lifespan. Trauma-informed care in all service settings reduces re-traumatisation, improves engagement, and produces better outcomes for people affected by trauma.
The Six Principles of Trauma-Informed Care
SAMHSA's trauma-informed care framework identifies six core principles. Safety: ensuring physical and psychological safety is the foundation of trauma-informed care — people who have experienced trauma need to feel safe before any other trauma-informed care principle can be effective. Trustworthiness and transparency: trauma-informed care involves clear, consistent communication and following through on commitments, rebuilding the trust that trauma damages. Peer support: lived experience expertise is central to trauma-informed care, recognising that recovery is facilitated by connection with others who understand trauma firsthand. Collaboration and mutuality: trauma-informed care flattens power differentials, recognising that healing happens in relationships and through genuine partnership. Empowerment, voice, and choice: trauma-informed care prioritises restoring the sense of control and agency that trauma removes. Cultural, historical, and gender sensitivity: trauma-informed care recognises that trauma is experienced differently across cultural contexts and that historical and collective trauma must be acknowledged.
